OK at idle/hot when driving? clogged radiator (internal core or external fins), clogged block.
Hot at idle/OK when driving? fan problems (U/S viscous coupling) or possibly waterpump.
Hot all the time? as for OK idle/hot running (but last stages before absolute failure) also waterpump.
Losing coolant? coolant/outside breach in head gasket.
Milky oil? oil/coolant breach in head gasket.
Bubbles in radiator when warm (warm vehicle with radiator cap off) cyl/coolant breach in gasket.
Start by doing the simple things first, look at the colour of you engine oil. Then warm the engine with the radiator cap off and look for bubbles. check that the fan operates as it should (electric or viscous).
Next would be to backflush radiator core and block, and make sure the radiator fins are clean.
Next would be to replace the waterpump, then finally look at doing the head(s).
Is there any more information you can give us about the problem?
