If you look at the photo you will see a spring on teh inside. This is teh return spring for the handbrake. The pivot is the point just below it and the cable joins on below that.

will take some pics/post my setup when i get the chance, works well has been on for about 2000km now with no probs (except i really need 16" rims because rocks get stuck between caliper and rim with 15")
Handbrake works fine too on this setup though i need to refix a couple of pivot mount which have loosened and made it a little less effective.

what gutsy said except use GQ rear disks then there is no need for spacers, just 8-10mm plate brackets, all bolts together. around $200 in parts, ie cheaper than fixing the disks. bit of work in the brackets. I can pull mine and take photos/drawings of the brackets of you want.
need to grind about 5mm out of the inside of the caliper bracket to clear the massive GQ disk.
