was the head crack tested?
was the cooling system bled properly?
could be somthing simple- may be the right time to upgrade er to a 4.2.

id say take it back to the mechanic who repaired your vehicle. get them to do a head gasket leak test and go from there. they can do this quickly with what we call a teekay headcheck which detects combustion leaks by changing the colour of a special fluid within the radiator filler neck.. works a treat and give a fairly accurate indication.
cheers

Your symptoms tell of a combustion leak into the cooling system. A teekay wont work on Diesels.
