you will have to go to a cnc engineer to get it made up
and will cost at least $1500 dollars

As sideways said, you will prob need to get it done custom.
If you want to give it a go yourself, especially if you are any good at 3d modelling programs, is to precisely draw both surfaces where you would either bolt up a sandwich plate if possible using original bellhousing, or remove bellhousing and draw the surface which it bolts up to.
Then you have to line up those drawings at the right distance from each other for splines to mate, etc....
It would be a pretty hard task, especially matching splines n flywheels/flexplates, etc...
Also, a lot of engineers or similar places would have a special device which can map a 3d object on computer by (basically) using an arm and a pointer to map certain points on whatever object you want to draw (like bolt holes, outside diameters, etc... which the computer then models.
