hilux cv problems

im in the process of rebuilding my hubs on my 88 hilux and in the process found something that i hope some of you guys can help me on.
soo i had undone the top hub bearing cap to remove the steering arm and was in the process of removing the manual locking hubs i started tapping the outside to try and loosen the collets and the whole hub falls off the knuckle. when i look inside i find a spread out circlip that had fallen off the axle and was sitting in the bottom of the knuckle and was extremely worn also there where wear markes around where the bottom bearing sits and on the cv itself where i found the circlip also the end of the axle was rounded where it looks like it has gone into the cv too far and was rubbing on the inside.
soo what i think has happened is the circlip has fallen off the axle allowing it to slide too far into the cv the circlip has been stuck between the cv and knuckle housing leaving wear marks on both.
my questions are does this sound possible?
what do you recon would cause this to happen as the groove where the circlip goes looks good and isnt rounded. would it just be bad luck?
is there anything i should do to stop it happening again?
and ive posted some picks soo you guys can have a look. but should i re use the cv or should i be replacing it because of the wear?
hope all this makes sense. any feedback would be appreciated.
