Depends on the level of boost. Biggest problem is soft heads, the alloy hardness deteriorates due to the heating and cooling over time. A rockwell hardness test on numerous locations of the surface needs to pass at least 85 Rockwell(bare minimum for NA), and ideally over 100 Rockwell is better(new is around 130.
Stock headgaskets that have a graphite coating are excellent as are multi layer steel shim as used on the 4.5.
