Clutch Problems

Hey all. I have been having a few problems with my clutch lately...
Whenever I push in the clutch pedal the clutch doesn't actually start grabbing untill it is about three quaters of the way down.
I have adjusted the adjustment bolt underneath the floor like the Instruction manual says to. But it keeps slipping out after a while and goes back to its orginal grabbing position. Which is GAY.
So What can I do to fix it?
