Loosing vacuum assistance - brake & clutch

Guys,
I seem to be randomly loosing vacuum assistance on brake and clutch in my car. Loosing the clutch doesn't really worry me but the brake does... Scary when approaching an intersection and the car keeps rolling cause I can't push hard enough...
Also scary on the weekend when reversing down a hill to snatch a mate up a ledge and the car keeps rolling...

It doesn't seem to have any order to when it fails ie. low/high revs. Seems to be random...

Does anyone know what it could be???

:T: a vax gauge in there and see when it goes, or leaks. There is a one way valve that 'may' leak. Also in the past I have put aux vac tanks on Patrol (petrol and diesel) to hold vac for those times you need to hold brakes ie. on leg shake hills.
