OH NO! CANT STOP

Had a bit of a problem last night. Jumped on the brakes and nothing happened! Had to use 1st gear and handbrake to stop. Just missed a brand new 5series BMW.
pulled in to a servo and pulled drivers rear drum apart and found that wheel cyclinder had pushed out a piston. shoes wernt completly worn out and wheel cyclinder was bolted in tight.
quick fix and drive home slowly leaving heaps of room.
now questions what would cause this??
whet is the maximum size of drum???
and what is the part number of the new replacenent shoes???
and lastly who does rear disc conversion kits and how much??
