2.8 TD cold start problem

My mates 2.8 turbo diesel pajero runs very rough, is very rattley and blows a heap of blake smoke when he starts it up cold. If he uses the hand throttle it runs fine and goes well when it's warm.
He had it tuned up a while ago with some more boost etc. He took it back 2 the diesel mechanic and was told they couldn't find anything wrong :?

The glow plugs are only a year old so there still in good condition.

Any help would be appreciated.

Your explanation is a little vague, but the 2.8 have a timing chain problem.
It can wear quite fast and it then sucks power from the engine.

It runs fine.
It's only when he starts it cold and lets it idle that it plays up.

Mate, I have a very similar problem with my triton 2.8 non turbo except mine blows white smoke on start-up. After a lot of screwing around with different mechanics I have narrowed the problem down to one (or a combination) of three things: injectors leaking fuel when engine is off, value clearances out, fuel pump incorrectly adjusted. I'll get them fixed when I get the cash. At the moment I am driving around with the hand throttle turned up to the correct idle speed (750 rpm).
