Low coolant level.
Blocked radiator (internal or external).
Blocked air con condenser fins.
Shagged water pump, the impeller blades can corrode.
Kinked hose.
Stuck thermostat.
Thoroughly check entire cooling system, flush and when fault has been repaired fill with water and additive.
Fan is only used at low speed, but should be in good condition as mentioned.

I just fixed the same problem on my 2.8D this monday! It only ever happened when it was under a fair bit of load/stress. (towing boat, and a/c on the highway up a hill). I replaced the fan, thermostat twice, fan belts, hoses, and it was still heating up. I was about to get into the water pump when it turned out the radiator had a blockage. Run your hand along the back of the radiator when the motor is warm and check for some inconsistant cool spots. Turns out I had a little blockage in the radiator and a tiny crack at the top corners under the top tank. Pulled out the radiator and got it re-cored. Took about two hours and $100. Has run fine all week towing the boat along the highway at 110ks. If your car doesn't get hot under normal conditions it can only be your radiator or thermostat. These things playing up will only really show itself under stress.
