yep it happened to me.

There was nothing left of the brake pads, and one fell out, causing the piston to over-extend resulting in loss of brake fluid = no brakes. tow truck ride home.

New brake pads all round & all fixed. Although since then I have adjusted wheel bearings. So not sure if that maybe contributed to me losing a brake pad as spike_sierra suggests...?

Yeah once in my old Sierra dropped out when I was reversing out of the car park one day, had to drive home in back streets going slow didnt know or think it was the brake pads though till when I got home and I went and looked around the car to work out my lack of brakes...

Then twice in my current Sierra one fell out of the rear in my current Sierra when I was dropping my mate home one day I knew what it was but we couldn't find it on the road any where. So I put it down to running the pads to low as the rear has no movement like the front does with worn or bad wheel bearings... This was a year or two back, then early this year on a muddy trip out another one fell out of the rear in some heavy mud action couldn't find it anywhere....
..wrench, wheel, wreck repeat..

check out the action @ http://www.darwin4x4.net

I've seen it happen. It was quite exciting as it was on a super steep descent. Check the discs are within spec and the anti rattle plates are installed between the caliper and the pad. They're only thin but you do need them.

They are stainless shims that sit between the piston/caliper and the pad.

You'll know they're there.
