I'd suggest that if all 4 came loose then they were fitted incorrectly or the torque specs they were tightened to were incorrect.
Maybe there was mud on the hub before the spacer was fitted... Maybe they weren't tight enough...
I've run 1.5" spacers on my Zook for about 18 months.. I check them regularly and have NEVER found one loose.

I ran them for 12 months permenently but I used locktight and they were just as tight when I removed them a year later.
