A month and a half ago ( roughly ) I had a bit of a noise from the front end of the Paj, rumbling sound, was on a sunday morn, So I pulled it down to take a look..
big chunk missing from the passanger side inner brake pad ... could not figure out the brand, had no idea how old they were so I got some new ones (the rest of the pad looked fine and had about 2/3rds of the friction materlal left on it )
So I got some new pads and (bendix 4wd ones .. with titanum stripe) and has not given me any issues since.
fast forward to saturday .. was working for a friend at a wedding (doing some bar work) was about 120k trip each way, almost there I hear a clunk from the drivers side rear .. stopped had a quick look, could not see anything so continued on ...
on the way home (1am ish) I hear a familiar rumble .. bloody pad has fallen to bits .. they were DBA pads on the back and appeared to be quite new when I checked em (after the front had an issue)
Due to the disance home I had to continue driving and have buggerd the disk surface a bit ( new disk is not to bad bout $100)
Now can anyone give me a reason for pads to fall apart like that .. apart from a manufacturing fault ?
I have no idea how old the were as they were on the vehicle when I purchased it in June. The other three pads are in good nic and less than half worn. I could see they were DBA brand on the rear, and the right part for the vehicle ..
The front I put down to just a "shit happens" type arrangement but for the rear to do it as well ... (inner pad again)

Pads can crumble if they get WAY hotter than their designed opperational temp range. Is there a chance that the slides on the calipers (i think the paj's have slide style calipers 
) are sticking you may have hte pad jamed up against the rotor, causing it to wear and overheat.

Yep that is a possability, the front rotor did not apper to have any hotspotting on it and I didnt pay alot of attention at the time .. just did the same things I normally do when replacing pad ( clean pins etc check piston blah blah blah) But on the rear I did notice the lower pins on both sides were very sticky and a bit of a bugger to remove. I cleaned the gunk off em (and the "tube" in whick they slide with a bit of parts cleaner and a tiny bottle brush thing ) Lighty lubed em up with a high temp copper based spray lube ..

Could just be poor bonding material gluing the pad material to the backing plate. I had that issue once on an XB, (pad fell apart just as you described) and it turned out it was a bad batch, thats all.
They were replaced for free.
Re above post, I thought brake pins should only have a special brake lube applied.... is that the copper based stuff you used?
