Kingy has been looking into this heaps, and im keen to do it to.
If u search there is a thread somewhere that had info in it on this.
i think it was called " Mq rear disks, whos interested " or something similar.
Basically, GQ rear disk brakes ( MQ disks in the front are the same as GQ fronts ) and pads and then the bracketing for the calipers etc. off the housing. A slight mod to the master cylender is need to i beleive.

Ive got vented disks on the rear of my mq, the job was done using tarago calipers and landcruiser disks, work was carried out before i brought the car by total 4x4 in epping, i rang and spoke to rod to see what parts were used in case anything needed replacing.
No brake proportioning mods were required and works well so far.

- in case you wondered, my car has flipped up u bolts, which is why the plates are on top of the axles....
The caliper has been ground a little also as shown in one of the photos.
Would have posted earlier but had to 'borrow' camera, computer etc.

Na its not sprung over, just flipped u bolts and reversed front shackles, stops those great big plates from digging into the ground.
May spring over one day, but it already runs 36 inch simexes off road and doesn't scrub (tray rear and gq gaurds) and I haven't had it out enough to find its limits yet, when i do I'll consider soa and larger tyres maybe.....

There is a reason ............. It's called brake bias !!
The proportioning valve does the job on ya rig now but its designed for rear drums.... By converting to Disc the proportioning is all wrong and will result in rear wheel lockups all the time. The valve is located on the inner side of the drivers side chassis rail, just in front of the bend before the rear diff. GQ's run a diferent system
Ways around this are......
Fit a GQ brake proportioning valve along with GQ rear discs & calipers
( pain in d butt system tho )
Fit smaller discs/calipers/brake pads but leave the std MQ/MK proportioning valve alone
Fit GQ discs & calipers but remove a percentage of the actual brake pad material ( not the backing plate ) & run the std MQ/MK proportioning valve
As for the actual conversion it's straight foward, it's getting the proportioning right that's the hard part.

From what i can see, the bracket is just 8-10mm plate cut to sit on the axle tube on one side and then cut so that the tarago caliper can bolt to it, the plate sits on the inside of the flange on the end of the axle tube, it is welded to the axle tube, and the holes in the flange are continued through the plate bracket so the axle retaining bolts can go through (seen in the photo)
As far as qg stuff goes, thats probably a logical choice, im not familiar with it though and don't know how the proportioning would go, i guess it comes down to the piston diameter and its radius from the axle.
But like i said my rear disks are from the front of a cruiser and the tarago calipers, and it hasn't had any proportioning mods and works fine....

what about a whole gq diff, same amount of work to weld on a few brackets for the 5 link, and coil hats! bonus flex too. then comes ratio's etc.
