Water intercooler popping

Hey all,
Just wondering has any one experienced a water to air intercooler popping, and sucking water into the intake? Does this happen?

My core is a few years old, no issues, PWR barrel.

It's not all that common. Has coolant been used? How old/what brand cooler?
