I get to change a tyre like that at least once a week, Its usually caused by gutter jumping at acute angles and highish speed. You can see a dent in the rim in the first picture. Looks like the tyre has shifted 90 degrees too.

But like you said, you wouldn't notice its down until the inner got too hot and blew out!
