help wheel bearings keep coming loose

well as above i changed my wheel bearings and now for some reason they keep coming loose as soon as a drive about 100 meters i cant work it out ive re done it about 5 times and the same thing any ideas please

What vehicle/model?
What sort of locking mechanism? (pix)
How are you setting the preload?
Do you have large offset rims on a nissan?

put the screws back in the locking tab?

Looked at the back of the knuckle as you check wheel bearing freeplay to make sure its not actually your swivel hub bearings which have the freeplay??
