Firstly there's no such thing as at your own risk anymore. These days with the smart arse greedy lawyers vying for every penny they can get nothing is ever your own fault. This is why as an organiser you need public liability insurance.
As far as PAI goes it's easy to get if you want to pay.
No standard personal insurance policy will cover you during competition, there is usually and exclusion written into the policy if you engage in dangerous activities. most insurance companies however will provide an extension to cover you for the respective activity if you ask for it. It will usually require a more stringent medical and be three or four times as much as your standard policy (depending on how often and what activity) but there is plenty out there.
As i mentioned, it is your responsibility to get your own insurance so don't blame event orgaisers for not providing you with any cover.

If competing in events i would run a six point cage as a minimium for your own saftey and just take the risk with the road but it all comes down to the construction of the cage on how strong it is to.
All event organisers have saftey as one of there bigest aspects, but medical help not just first aid certificate would be and advantage to all involved(due to how remote most events are) if something bad was to happen and i think majority of compeditors and spectators would be happy to pay higher admission for these services.
